Job Description Position SummaryThe Evaluator is responsible for leading the program’s performance measurement, outcomes evaluation, implementation assessment, and reporting activities. The position will design and oversee evaluation methodologies that assess clinical outcomes, participant engagement, operational performance, healthcare utilization, and workforce-related impacts associated with remote patient monitoring and chronic disease management interventions. The Evaluator will ensure that findings are translated into actionable recommendations that support continuous quality improvement and long-term sustainability.
Essential Duties and Responsibilities
  • Develop and maintain a comprehensive evaluation framework, logic model, and performance measurement plan.
  • Design quantitative and qualitative evaluation methodologies to assess implementation effectiveness and program outcomes.
  • Establish baseline measurements and monitor progress toward defined performance targets.
  • Analyze clinical outcomes including glycemic control, blood pressure management, weight change, and participant engagement.
  • Evaluate healthcare utilization outcomes including emergency department visits, hospitalizations, and readmissions.
  • Assess participant satisfaction, provider satisfaction, retention, and intervention fidelity.
  • Conduct comparative analyses between intervention and comparison cohorts.
  • Develop dashboards, scorecards, and executive reports for leadership, stakeholders, and external reporting requirements.
  • Collaborate with clinical, operational, and technology teams to ensure evaluation activities are integrated into program workflows.
  • Lead continuous quality improvement reviews and root-cause analyses when performance falls below established targets.
  • Support dissemination of findings through presentations, manuscripts, reports, and conference submissions.
  • Ensure compliance with applicable privacy, regulatory, and research standards.

Knowledge, Skills, and Abilities
  • Program evaluation and outcomes assessment.
  • Statistical analysis and interpretation.
  • Health services research methodologies.
  • Quality improvement and performance management.
  • Technical report writing and presentation skills.
  • Experience using analytical software such as R, SAS, SPSS, Power BI, or Tableau.

Required Qualifications
Education
  • Bachelor’s degree in Public Health, Epidemiology, Health Services Research, Biostatistics, Evaluation Sciences, Health Administration, or related field.

Experience
  • Three years of experience conducting program evaluation, health outcomes research, or healthcare analytics.
  • Demonstrated experience with statistical analysis and evaluation methodologies.
  • Experience preparing reports for healthcare organizations, government programs, or grant-funded initiatives.
Preferred Qualifications
Education
  • Advanced degree (Master’s or Doctorate) in Public Health, Epidemiology, Health Services Research, Biostatistics, Evaluation Sciences, Health Administration, or related field.

Experience
  • Experience evaluating digital health, telehealth, remote patient monitoring, or chronic disease programs.
  • Experience with implementation science frameworks.
  • Familiarity with value-based care and population health measurement.
