Position Summary	The Health Educator/Navigator leads the education and lifestyle programs focused on preventing or delaying the onset of type 2 diabetes. This role coordinates and delivers the adapted Diabetes Prevention Program with genetic counseling and family skills building. The ideal candidate uses effective communication skills to guide participants toward healthier habits, track program outcomes, and support high-risk populations.
Minimum Qualifications	Education: At minimum, a bachelor’s degree in Health Education, Public Health, Nursing, Nutrition, or a related field from a regionally accredited college or university. Skills: Strong public speaking, group facilitation, and interpersonal communication skills. Experience: 1-2 years of experience in health promotion, community outreach, or patient education. Education: Associate degree or two (2) years of college required. Instead of a degree/college, at least five (5) years of experience as a Community Health Worker. Experience: Minimum of one (1) year of clerical experience in a community health program, to include experience utilizing a personal computer. Ability to communicate effectively with research participants, community contacts, and staff both in writing and orally. Experience with Microsoft Word and Excel
Preferred Qualifications	Training: Formal training or certification in diabetes prevention (such as a CDC-recognized Lifestyle Coach training or Certified Diabetes Care and Education Specialist credentials).

Job Duties

Description of Job Duty

Lead Education Sessions: Teach structured classes on genetics by way of understanding family health history, healthy eating, physical activity, weight management, and stress reduction. Deliver Curriculum: Facilitate the approved diabetes prevention curriculum with fidelity to program guidelines. Assess Risk: Evaluate participant health behaviors, track weight loss, and monitor physical activity logs. Provide Counseling: Offer supportive coaching to help participants set and reach realistic health goals. Engage the Community: Partner with local organizations and host outreach events to recruit eligible participants with prediabetes. Manage Data: Record attendance, retention rates, and participant progress metrics accurately for reporting and grant compliance.
