To provide social worker services to an assigned population in support with mental illness, chronic disease and appetitive drive disorder and their support network, helping them by providing integrated care to reduce harm, while also serving as a representative of the field of Social Work.
Description of Duties
•Provides behavioral health counseling services to patients at the UT Health RGV Pediatric Specialty Clinic as determined by General Pediatrician.
•Provides psychotherapeutic interventions that reduce harm to patients who experience mental illness, chronic medical illness, and an appetitive drive disorder (substance abuse, alcohol abuse, eating disorders etc.) including psychotherapy, care coordination, information and referral, and follow-up monitoring.
•Documents information accurately and timely in the electronic health record.
•Collaborates with patients, the inter-professional team and patient support systems in treatment planning and supports the patient in transitioning from the clinic to long-term care.
•Serves as a member of a diverse, inter-professional team and professional/graduate students and learners.
•Provides education on social services and public systems to professional/graduate students and learners.
•Trains and supervises graduate students in the fields of social work, counseling and psychology.
•Screens, assesses, and secures background information from patients and their families as part of the clinic new patient intake process.
•Documents information accurately and timely in the electronic medical record.
•Shares information obtained in patient and family interviews with physicians, nursing staff, and other clinic and community personnel as necessary for continuity of treatment.
•Engages in outreach and training activities with hospitals, mental health providers, social service providers, and other community partners.
•Initiates frequent contact with local and state agencies.
•Participates in program evaluation activities, including but not limited to data collection, grant reporting, patient satisfaction surveys, preparation and publication of peer-reviewed manuscripts, and other activities as needed.
•Performs related duties as required.

General direction from the assigned supervisor.

Direct supervision of all assigned staff.
Required Education
Master's degree in Social Work from an accredited university.

Licenses/Certifications
• Current Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) by the Texas State Board of Social Worker Examiners. • Preferred: Licensed Chemical Dependency Counselor (LCDC) certification issued by the Texas Licensed Chemical Dependency Counselor Program, a part of the Licensing and Certification Unit of the Department of State Health and Human Services.
Required Experience
One (1) year of professional social work experience.
Preferred Experience
Experience with severe mental illness (SMI) population and substance abuse.
Equipment
Use of standard office equipment.
Working Conditions
Needs to be able to successfully perform all required duties. Usual office environment.
